- Requesting agency & caller's name
- Agency phone number
- County in which accident is located
- Nearest major highways and intersections
- Nearest town & the direction and distance from the town to the accident.
Additional Information necessary to continue flight:
- Landing Zone (LZ) location in relation to the accident site.
- Latitude and Longitude of the scene, if known.
- Call sign and radio frequency of the unit in charge of the LZ include PL tone for your frequency
- Hazards associated with the LZ: power lines, towers, guide wires, posts, signs, trees, loose debris.
- Is there a HAZMAT situation?
