• Do you have questions about how to prepare for your adult life?

If you answered yes, then you may benefit from the services offered by CYACC…

CYACC Goals are:

• To help youth ages 11-22 with special health care needs look at both their current situation and their future goals.
• To help youth and families find the services and community resources they need to achieve their goals.
• To work with schools, community programs, primary care and subspecialty doctors, and other services to coordinate the care each youth receives.
• To work with adults with developmental disabilities and their families in maintaining their health and community activities.

CYACC is supported by IU School of Medicine, IUMG-PC, the ISDH Maternal and Children’s Special Health Care Services and the Indiana Division of Disability, Aging and Rehabilitative Services. Staff include personnel from About Special Kids and Indianapolis Resource Center for Independent Living.
