  • Do you have questions about how to prepare for your adult life?

If you answered yes, then you may benefit from the services offered by CYACC…

CYACC Goals are:

  • To help youth ages 11-22 with special health care needs look at both their current situation and their future goals.
  • To help youth and families find the services and community resources they need to achieve their goals.
  • To work with schools, community programs, primary care and subspecialty doctors, and other services to coordinate the care each youth receives.
  • To work with adults with developmental disabilities and their families in maintaining their health and community activities.
