How to Make a Referral

A referral is not necessary to be seen in our clinic. We schedule appointments directly with the patient's caregiver. All additional information from current mental health and primary care physicians are welcome. Please fax information to (317) 278-0609.

Caregivers, please call (317) 274-8162 and press Option 1 to schedule an appointment. Our current wait list is at least six months.
